You either get let out with a loss, or they give you a few minutes to let you finish with heavy assistance. They'll watch you and give you vague hints throughout the regular one as well, so that you'll likely be not too far off by the end. Some places also don't lock the door, as the challenge in some of them is the puzzle inside, not opening the door.
I've only ever seen the locked door ones
Some of them say "locked in" but only mean it psychologically.

No, thankfully I don't have to use them yet. But I do watch a little TV, a very little but still some so knew they existed. C'mon @Abe, you didn't laugh even just a little when you saw that post?
